OK guys for my part I needed to develop a complete care plan for hypernatermia and hyponatermia.

And I will need your help. So can you guys help me go through what we would put in a care plan.

We have a Patient that presents with   "A 68 year old women complains of being very lethargic,  very weak,  increasing irritability, small amount of edema, excessive thirst, nausea, vomiting, and loss of appetite."
Old bat who doesent drink enough

A 75year old man complains of having headaches, being confused, increased fatigue, restlessness and some spasms and cramps.
